Program: Wazee House AODA Residential Treatment Program – Black River Falls, WI
Program Description:
The program provides services to adult males who are referred through the Department of Corrections and who have a history of substance use disorder. The program is licensed as a Community Based Residential Facility (CBRF). The length of stay is determined by client need. The program utilizes evidence based practices targeted to reduce recidivism and relapse.
Job duties include but are not limited to:
• Assessing, diagnosing, treating and counseling clients.
• Work with clients to help develop their individualized treatment plan and define long term and short term goals.
• Documentation of clinical services delivered
• Attending staff meetings and trainings.
• Assessing of client problems and capacities, formulate individual treatment plans to meet client limitations (limited reading and writing level, ADD/ADHD)
• Provide treatment counseling individually and in a group setting while adhering to the evidence-based programming to address criminogenic needs which include: Substance Abuse, Anti-Social Thinking, Anti-Social Personality, Family & Relationship, and Anti-Social Peers (cognitive based program)

Job duties include but are not limited to:

Note: Diversion is a mental health care facility that specializes in client crisis situations. The facility aids in medication stabilization, maintain of personal safety, offers support to those who have a variety of mental illnesses which include AODA issues, assisting client in returning to their baseline, assist in obtaining community supports, and work with client to provide essential follow-up care.
Job duties (but not limited to):
• Initial intake of clients: Intake paperwork, creation of Individual Service Plans, maintenance of Medication Administration Records, evaluation of the client needs and assist in connection of services. Work in collaboration with the Brown County Crisis Center staff workers and country employees including but not limited to; nurses and psychiatrist, police officers, probation and parole officers, emergency medical technicians, and members of the fire department.
• Discharge Clients: completion of all discharge paper work which includes ; discharge instructions and/or recommendations, following the company medication protocol, ensure the clients (guardian or case manager) receive the proper information about their medications and scripts, return all personal care items and clothing, and assist them in obtaining safe transportation.
• Medication distribution: ensuring the proper protocol is followed when distributing the medication (dosage and documentation), reordering of medication, obtaining scripts, understanding of the different classifications of medication and following of all county, state and federal guidelines.
• Personal one-on-one time with the individual clients and assisting with their unique needs, prevention of crisis situations and de-escalation crisis circumstances, aiding the client in obtaining their personal goals according to their Individual Service Plan.
• Daily staffing: completion of shift report to incoming staff (includes: behavioral issues, medication changes, passes and where the clients will be going to their appointments), completion the of the doctor’s staffing book which summarizes the clients progress within the past 24-hours to aid the doctor in understanding of the client’s needs.
• Maintaining proper sanitation of the facility, preparation of the clients’ meals which include monitoring the federal guidelines that include special dietary needs of those who are diabetic, restocking the facility supplies that are required for daily activities, and following all county, state and federal guidelines of proper facility and client care.
• Detoxification: social detoxification of referred individuals, monitoring of vitals and recoding signs or symptoms of alcohol withdrawal, and following the facility protocol to ensure the safety of the client, working with the county employees to ensure proper placement if needed upon discharge.
