Caregiver

Duties:
? Supports patients by providing light housekeeping and laundry services;
shopping for food and other household requirements; preparing and serving
meals and snacks; running errands/doctor�s appointments.
? Competently assists patients with Activities of Daily Living (ADL�s) by
providing personal services, such as, bathing, dressing, and grooming.
? Helps patients care for themselves by teaching use of cane or walker, special
utensils to eat, special techniques and equipment for personal hygiene.
? Records patient information by making entries in the patient journal; notifying
nursing supervisor of changing or unusual conditions.
? Providing medication reminders as directed by Nurse Field Supervisor and plan
of care
? Assist bedbound clients in changing positions to prevent ulcers
? Demonstrate ability to effectively communicate with client, family members,
and 5 Star Care Team.
? Report all appropriate occurrences to office staff and/or Field Nurse Supervisor
? Maintains a safe, secure, and healthy patient environment by following asepsis
standards and procedures; maintaining security precautions; following
prescribed dietary requirements and nutrition standards;
? Protects the home care agency by adhering to professional standards, home care
policies and procedures, federal, state, and local requirements.
? Enhances service reputation by accepting ownership for accomplishing new
and different requests; exploring opportunities to add value to job
accomplishments.
? Consistently demonstrates professionalism and follows through on the
responsibility of the assignment
? Maintains confidentiality of the client PHI, observations and records. Respects
the rights, privacy and property of others at all times.
