The L.I.F.E Group House Manager position is a front line position that provides residential oversight, staff (specifically DSP) supervision, and direct support for individuals with intellectual disabilities and behavioral challenges who reside in 6400 licensed community residential homes. The supervisor is responsible for the supervision of their staff, Direct Support Professionals, attending medical, dental and psychological appointments, and for the assessment, planning, implementation, monitoring and evaluation of daily living skills, documenting and addressing all site needs, scheduling all activities, developing weekly menus and the maintenance oversight of the agency vehicles and all agency as well as governing stakeholder required documentation. The supervisor conducts all program operations in accordance with the principles of Everyday Lives, Normalization, abiding by DPW 6400 regulations and with sound ethical work performance. It is expected that the supervisor will act in accordance with the policies and procedures of The L.I.F.E Group at all times. Accordingly, a supervisor will be stationed at his/her respective sites functioning as a support to both individuals and DSP’s to ensure compliance with internal and external agency guidelines.

A) Operational Responsibilities:

Responsible for the overall daily operation of the home.
Responsible for finding coverage of home when callouts occur. Management must get approval for staff working over 40 hours. If coverage is not found, the supervisor is responsible to cover any callouts or vacancies not covered by DSP.
Ensures that all care and services to individuals are provided in accordance with state and county regulations.
Ensure that all individuals are active in program activities and provide feedback to The L.I.F.E Group Leadership team
Provide daily support and guidance in establishing direction and standard of care as outlined in the DPW 6400 regulatory guidelines.
Assists with maintaining household budgets, including making purchases for both the needs of the individual and household supplies and distributing funds for social/recreational activities, monitor overtime hours, utilities, repairs and financial reconciliation.
Collect all receipts from staff and/or individuals after outings and/or purchase are completed and submit to supervisor monthly so funds can be reconciled.
Ensure that monthly fire drills are conducted on alternating shifts and documentation is maintained.
Reports all maintenance issues.
Maintain on-call status, which include but not limited to being available 24 hours per day, seven days per week by cell phone, notifying Assistant Director and/or Director of any and all major incidents involving staff and/or individuals and insures required documentation is completed.
Accompanies individuals and DSP’s on medical and dental appointments as deemed necessary.
Follow-up with pharmacy regarding prescriptions updates.
Attends all management team meetings, clinical consultation meetings and communicate all issues and/or concerns.

B) Administrative Responsibilities:

Ongoing review of medication logs to ensure proper documentation and administration of medications.
Assists Clinical/ Program Specialist in assessing the status of individual’s programming, including goals, activities of daily living, and community base integration and CPS participation.
Review and approve DSP’s time sheets.
Maintain staff schedules, providing daily staffing, based on the individual ratio for those served.
Ensures that information about the progress and status of the individuals are accurately and appropriately communicated.
Works closely with the administrative team to ensure the best program option for every individual is communicated.
Report on and follow-up with all medical/behavioral issues with Medical Coordinator and/or Assistant Director.
Review and make all necessary corrections to Medication Administration Record (MAR) from the pharmacy prior to use at the beginning of the new month or any additional request given by the prescribing doctor, only if you are Medication Administration certified.
Semi-weekly checks of all documentation, i.e. progress notes, individual outcomes, CPS, weekly menus, incident reports, medication logs, daily notes, etc.
Assist with incident report closure by submitting requesting documents to the appropriate management team member.
C) Staff Development and Supervision:

Supervises and evaluates the performance of DSP’s.
Address staff performance concerns and take necessary disciplinary action in consultation with the Assistant Director.
Document absenteeism and tardiness of DSP’s.
Conducts On-Site Orientation with new hires.
Conducts monthly staff meetings and forward copy of meeting minutes to Assistant Director.
Insures staff compliance with all programs and L.I.F.E Group policies and DPW 6400 regulations.
Promotes and ensures effective communication among DSP’s.

D) Vehicle Maintenance:

Ensure the maintenance of vehicle mileage logs in the program vehicle.
Ensure that updated vehicle registration and insurance information are in the program vehicle.
Contact the Assistant Director for regular maintenance, oil changes, repairs, and inspections for vehicles and keep an accurate record of it.
E) External Relations:

*Acts as unit representative at community meetings and other public venues as necessary and in accordance with The L.I.F.E Group vision and mission.

*Maintain effective working relationships with personnel from the county, local agencies, support coordinator and families to coordinate service delivery to individuals.
