Case Manager
This position is grant funded.
The primary focus of this position is assisting individuals who have experienced homelessness in obtaining and maintaining housing.
Basic job duties include:
1. Home visits
2. Developing individual housing support plans
3. Provide crisis intervention services focused on enhancing the clients� ability to:
a. independently problem solve,
b. use effective coping skills,
c. manage and self-coordinate own care
4. Participate in community meetings as required
5. Maintain data and documentation for grant reporting
Hours are typically Monday - Friday 8a-4p, with some pre-planned scheduled changes on an as needed basis.
Pay is from $15 per hour
