We are currently looking for a compassionate, energetic, and creative individual to join our team. Our Client Relations Specialist schedules, plans, and executes all visits including initial client assessments and home visits. The CRS also maintains and strengthens relationships – both with individual clients through monthly home visits and with referral sources through follow-up visits and phone calls.

Duties and Responsibilities:
• Executes all aspects of the initial client visits and intakes including scheduling, client profile creation and
communication of the information to the Care Coordinator.
• Executes monthly home visits to existing clients.
• Maintains relationships with existing Referral Source/Clients. Visits and calls regularly.
• Works closely with the Care Coordinators and the Caregiver Coordinators to create a complete, successful, satisfying, and on-going life cycle of exceptional services to our clients and Caregivers.
• As available, attends and helps the Community Relationship Coordinator with various private and community-based programs/events run throughout the year.
• Utilizes Outlook for email and calendar appointment/event scheduling.
• Must be able to see and interpret information within our systems and be able to exchange information accurately and quickly with others, as well as record information in these same systems.
• Must have a valid driver’s license, positive driving record, reliable transportation and must be able to drive to and from client sites for various reasons whenever necessary and at a moment’s notice.

• Experience in the healthcare field is ideal.

Educational Background/Prior Experience:
• Human services background or strong desire to help others
• High School Diploma - BA/BS related field is a plus
