Case Manager

A general description of the Non-Medical Case Management position is that we coordinate, monitor, and facilitate access to a broad range of HIV-related health and human services. This position would be based out of our Satellite office in Elkhart, serving roughly 65 clients with about 30% being Spanish-speaking only. The ultimate goal for NMCM is to assist the client towards self-sufficiency. There are few restrictions, but some of the main responsibilities include connecting clients with medical providers, applying for internal and external financial assistance, and offering social support and encouragement. The training period is 4 weeks long, and you would mostly be working with the Non-Medical Case Manager Lead during this time. You would also have the opportunity to shadow any of the other Case Managers as they conduct appointments. I do not have a set training schedule, as I prefer to tailor training to fit the learning style of the employee. The training will also consist of 3 mini exams (typically one/week) plus a comprehensive exam at the end of the 4 weeks. We do have an agency van however, if you were to use your own vehicle we do offer mileage reimbursement.
