We are a community-based hospice, providing quality end-of-life comfort care to patients in their home of choice, as well as providing in-patient hospice care in hospitals and skilled nursing facilities when needed. Our staff is committed to assisting families to understand the end-of-life process, educating them on symptoms to expect, and care that is provided, and empowering them to participate in providing care for their loved one at the end of life.

PURPOSE OF POSITION: Hospice Social Workers plan, organize, develop, and direct all required social services to meet the needs of the patient and their family in accordance with federal, state, and local standards, guidelines and regulations and ensure that the psychosocial and emotional needs of the patient are met. They provide care in private homes, residential care facilities, nursing centers and other hospice care environments. This position will start out as Part Time and will grow as our agency grows to Full Time as our patient needs increase.
ESSENTIAL FUNCTIONS: · Visits Hospice patients and their families in their home of choice to meet their psychosocial needs at end of life. · Conducts initial and ongoing assessment of patient/family’s psychosocial history and needs. · Participates in developing the interdisciplinary plan of care by formulating psychosocial goals and interventions that meet the individual needs of each patient. · Assists members of the Interdisciplinary Team to understand the significant psychosocial concerns related to the patient/family’s response to dying. · Collaborates with other members of Interdisciplinary Team to facilitate care conferences and family meetings. · Acts as a liaison to outside agencies to assist patients and families to access needed programs and community services such as Meals on Wheels, private home care services, etc. · Participates in bereavement discussions, assesses bereavement risk for all patients and families and facilitates group support meetings. · Assists in developing and presenting educational and public relations programs for staff, our care partners, and the community at large. · Maintains current documentation in patient records of services provided and contacts made.
EXPERIENCE AND QUALIFICATIONS:Masters’ Degree in Social Work. Licensed Social Worker in the State of Rhode Island with either LCSW or LICSW. At least one year of experience working in a medical social work setting. Understanding of and commitment to hospice philosophy of care. Openness in working with diverse religious, cultural, and ethnic populations. Ability to work effectively as a member of the Disciplinary Team
