Patient Care Coordinator

IL licensed and Medicare certified Home Health Care agency with strong ties to the local community is looking for an outgoing and hardworking professional with marketing skills and home health care knowledge to promote our services to the Hospitals, Skilled Nursing Facilities, Independent & Assisted Living Communities, Physician’s offices, and other patient care referral sources and prospects. This persons’ primary role will be to develop strong relationships with patients referral sources, and to coordinate and support patient's smooth transition into our care upon the discharge of such patient from medical facility. We are highly skilled agency providing medical care at the patient's home, including nursing, physical, occupational and speech therapies, as well as podiatrist, medical social workers, labs and CNA services. Promoting a positive image of our business in the community while representing us at community functions and in professional organizations is critical.

Qualifications and Skills:

Result oriented
Strong in-person and phone communication skills
Strong relationship marketing, networking, and consultative selling skills
Willing to follow a proven sales / marketing system
Excellent time management and organizational skills
Team player yet able to work independently
Strong attention to detail pertaining to documentation and follow up

Required Experience:

- Sales/marketing experience in health care is preferred but not required.
