PRIMARY RESPONSIBILITIES:

Meal preparation clean up
Provides medication reminders
Light housekeeping, laundry
Works towards goals for ILS/IHFS clientele
Provides patient transportation to Dr. appointments, community activities shopping
Documentation

REQUIREMENTS:

Valid drivers license, car insurance and reliable vehicle.
Diploma or GED
Authorized to work in the U.S.
