Care Manager

The care manager is responsible for managing the clinical and medical care for clients to include vulnerable adults and elderly populations. This position is also expected to support the overall operation of The Option Group when needed.

Care Manager conducts home visits, phone calls, virtual calls, and regular in-person visits with clients to gather information, assess conditions, living situation, and make recommendations.

Connect the evaluations and clinical assessments, to discern the situation and make recommendations.

Develop appropriate service plans with individuals, families, and service providers in accordance with regulatory agency policies.

Advocate for client to get services, entitlements, or the best care scenario for the client.

Connect clients, families and/or representatives to appropriate services and resources. 

Accurate document client files and maintain compliance with local health departments. Department of Aging and enrolling individuals in qualifying programs.

Knowledge of resources to include Medicare, Medicaid, insurances, local/regional facilities, services, practices, and community resources. 

Strong attention to detail and organizational skills, with the ability to prioritize and complete multiple work assignments on time.
