Preferred masters in social work or counseling, or in progress with behavioral health experience. Preferred active AZ licensure for social work or counseling.
Primary Care
Canyon Pediatrics is an integrated care provider looking to support patients with breif clinical interventions, screening/assessment, care coordination activities that include general referral follow ups, social determinants of health, and care coordination with behavioral health activities.
Specialty Behavioral Health Services
Canyon Pediatrics is working to increase access to specialty behavioral health services such as counseling, family support, and psychiatric services. This part of the position would look like more traditional therapy/counseling services for patients who have behavioral health needs.
Canyon Pediatrics & Family
We see patients from all demographics including AHCCCS, Commercial, and sliding fee. Canyon Pediatrics has been an integrated provider of services for over 15 years employing behavioral health staff to support medical staff in care coordination and treatment of children. In addition to fee for service Canyon also participates in value-based incentives with health plans and other entities.
We work to make community engagement, patient-centered practices, and communication a center of our practice. Applicants should be interested in a team or health home approach to pediatrics and willing to work alongside behavioral health staff both internally and externally.
