Family Resources Coordinator

Duties, Accomplishments and Related Skills:

The Family Resource Coordinator (FRC) provides administrative support to the Pediatric Therapy Early Intervention program.

The goal of the Early Intervention program is to improve the social, physical, and psychological functioning of children from birth to age three, including providing the family with support and resources.

This position is responsible for administering the client referral and intake process, connecting clients with community resources, and ensuring compliance with contractual requirements.

This position also provides supplemental administrative support to the Pediatric Therapy department and outpatient clinic.

ESSENTIAL FUNCTIONS: Administer the Early Intervention client referral and intake process, Process and track incoming and outgoing referrals Travel to client locations to administer intake process and paperwork, ensuring completion of all requirements Facilitate department wide case management meetings Connect clients with community resources Facilitate collaboration and communication between professionals and parents involved with the child and family Provide referrals to appropriate resources available in the community Ensure contract compliance Maintain, report, and deliver school district counts, Ensure early intervention client timelines are met, ESIT, count and school district paperwork submitted as required Serve as liaison to billing team Additional administrative support as needed.

This could include, but is not limited to: Scanning and uploading therapist documents and protocols in to EMR system Laundry services for the department Sending and receiving of faxes Provide backup to Scheduler as needed GENERAL RESPONSIBILITIES: Data entry with a high level of accuracy and efficiency within multiple software programs Maintain a high degree of client and employee confidentiality under HIPPAA standards Provide a broad range of support to program management and employees Attend and participate in meetings and trainings as directed Adhere to safe work practices and procedures COMPETENCIES: Intermediate to advanced computer skills in a Windows and Office 365 environment Organizational efficiency Clear and collaborative communication Sound judgement in ambiguous situations Ability to juggle competing priorities Detail and task oriented Ability to self-manage timelines and workflow WORK ENVIRONMENT: Indoor clinic setting with ambient room temperature; lighting; moderate noise typical of children’s activities and communication; equipment traditional of an office or therapy clinic setting.

7. Discuss progress of the children with their parents and respond to the expressed concerns of parents by designing individualized educational plan.
8. Develop a partnership with participating parents in which the strengths and needs of the family will be assessed and an individualized family services plan will be developed, implemented and documented.
9. Serve as a resource and support in identifying and accessing appropriate community resources for children birth to three years of age and their families.
10. Model appropriate communication skills, guidance techniques, and nurturing qualities and promote positive attachments between caregivers, children and families.
11. Provide consultation and support regarding best practices in early childhood education to providers of childcare for participating children.
12. Help transition families from Early Head Start to the next learning environment in the continuum of early childhood services.
