Private Duty Nurse prior authorization RN. Duties include evaluation of PDN request for Superior Health plan, through assessment of member’s medical, nutritional, and behavioral needs. Work directly with agencies, Physicians, and professional behavioral and nutritional healthcare providers.
January 2018-November 2018: Star Kids Service coordinator II, Level 1, Superior Health plan:
Care coordination for case load. Completing Annual, quarterly and PDN assessments, evaluating requests and determination of appropriate services based on need.
Evaluation of members medical and behavioral health needs based on nutritional assessment, current ability to complete activities of daily living, and diagnosis. Assisting members with acquisition of services, DME and community resources. Accurate documentation, coordination with team lead and supervisor. Monthly contact and follow up with members.
