Assisting physician practices in managing high risk patients by providing education, resource information, guidance and overall care management to this challenging population. Also identifying patient who may qualify for the CMS Bundle
Payment for Care Improvement (BPCI) program, providing them with diagnosis related education and assigning them to care managers who works with them on identifying health care goals for a 90 day period after hospitalization.
