Ambulatory Nurse Care Manager - 2015 - present
Providing care management services to the hospital community through the Accountable Care Organization (Quality Health Alliance). Assisting physician practices in managing high risk patients by providing education, resource information, guidance and overall care management to this challenging population. Also identifying patient who may qualify for the CMS Bundle
Payment for Care Improvement (BPCI) program, providing them with diagnosis related education and assigning them to care managers who works with them on identifying health care goals for a 90 day period after hospitalization.

Care Manager - 2007 - 2015
RN Care Manager at acute hospital level of care with experience serving on the stroke, medical/surgical, and cardiac progressive care units. Duties include daily utilization review, assessing patients for home care and durable medical equipment needs. Plays a key role on the interdisciplinary team of physicians, nurses, social workers, physical/occupational/speech therapist.
