Registered Nurse
RN to perform patients' assessment and obtain vital signs for start of care and every nursing visits. RN to create a plan of care and medication profile of the patient and review plan of care and medications with patients/caregivers upon admission. RN is to travel to patients' home and provide services and helping patients to maintain their independence. RN to educate patients and their caregivers on medication administration, discuss side effects of medications, perform wound care and teach Patients/caregivers on how to perform wound care, and updating doctors about their patients' health and progress. RN to report any change/changes in the patients' condition to their primary care physicians.
