SUMMARY: Responsible for the new client intake process and evaluating for additional needs. Responsible for obtaining, maintaining contact, information, and progress notes on clients. Provides limited social work services to clients.

DUTIES AND RESPONSIBILITIES:

Provide services in accordance with our mission statement.
Responsible for adhering to all policies and procedures.
Responsible for initial referral, assessment and coordination of meal service for every client.
Collaborate with other professionals to evaluate client’s medical, physical or emotional needs and to assess client nutritional needs.
Perform telephone and home visits.
Monitor, evaluate, and record client’s nutritional needs. Re-evaluate every six months to determine changes in diet or living conditions.
Document modifications in nutritional plans.
Identify environmental impediments to client through interviews and volunteer observation following meal deliveries.
Contact client following no response at the door during a scheduled meal delivery.
Counsel family members or emergency contacts to assist them in understanding, dealing with, and supporting the client.
Utilize consultation data and social work experience to plan and coordinate client nutritional care, following through to ensure service efficacy.
Oversee Medicaid- and Medicare-related paperwork and record keeping as necessary.
Investigate neglect cases and take authorized protective action when necessary.
Share in the responsibility of general office tasks
Prepare, pack, drive and deliver meals when necessary.
Assist in fundraising efforts and activities.
Perform other duties as assigned by the Executive Director
MINIMUM JOB REQUIREMENTS:

Bachelor’s Degree in Social Work with focus on Eldercare or related field. Minimum two years experience. New graduates with internship training in related field may apply. Equal Opportunity Employer.
