We are a non-medical, in-home caregiver agency that is dedicated to those we serve and to our team who makes it all happen. Our services include personal care assistance, meal planning and preparation, medication reminding, housekeeping duties and other tasks that allow our clients to remain healthy, happy, and home.

We are seeking a dynamic individual to fill our Care Manager role to help us continue to provide GreatCare® to the seniors in the Fremont Community. The position is a full-time salaried-exempt management position that requires the ability to also be available for on-call rotations. Ideal candidates must have a passion for working in the Eldercare Industry, is dedicated to making a difference, highly organized has experience in Senior Care.

Primary Responsibilities:

Interviewing/Hiring Caregivers
Schedule all client/caregiver shifts
Matching caregivers with clients
Consulting with clients (and families) for needs assessments
Conducting Caregiver Orientations to assigned clients.
Conducting onsite Supervisory visits
1-on-1 client interactions
Providing exceptional customer service to clients and caregivers
Accurate/timely documentation and management of client/caregiver management system
Travel to client homes is required.
Working Conditions:

The Care Manager will spend hours in the office and in the client’s homes. The Care Manager will be required to do some lifting as necessary. The position will be based in a busy office where s/he will be faced with constant interruptions and must meet with others on a regular basis. The individual in this position will deal with a wide variety of individuals and client/caregiver issues on a daily basis.

Qualifications:

Minimum 1-year experience or certified training in care for the elderly or individuals with functional disabilities
Minimum 1- year of staffing experience in a healthcare setting
18 years of age
Speak, read, and write English (bilingual a ++)
Reliable transportation
Ability to pass federal and state background check
Clean MVR (motor vehicle record)
Possess a valid driver’s license
Current CPR/First Aid Certification
Bachelor degree preferred, experience in the field will be taken into consideration
If you have a passion for customer service and understand the importance of excellence in your work, then you may qualify for the role. The ideal applicants will have experience in caregiving and are individuals that have experience in managing others in a healthcare setting such as a home health company, professional medical practice or a facility that provides services to elderly. Those individuals that have an education in Healthcare management may also qualify without direct experience. Salary: $35,000 - $40,000/year + bonus potential
