§  Completing and Render services to adults, interventions, and skill-building activities in accordance with the Individualized Service Plan (ISP) to meet the goal of minimizing the negative effects of psychiatric symptoms or substance dependence that interferes with the consumer’s daily living and personal development.

§  Contributes to the development, implementation, monitoring, and updating of the individualized service plan.

§  Conducted and Review assessments and consumer service plans or treatment plans to ensure that appropriate services are provided to meet the needs of the consumers being serviced.

§  Responsible for understanding the consumer’s needs and providing coaching, so that a trusting relationship can be established with the consumers.

§  Responsible for building a trusting, coaching, and understanding relationship with the consumers, and consumer’s guardians involved on his/her caseload to ensure a positive outcome with the behavior/symptoms.

§  Responsible for conducting group and individual psychotherapy Session's weekly a on an intense basis and utilizing focus treatment strategies.

§  Provide Document all counseling and education services, assessments, reassessments, referrals, and follow-ups in the consumer's clinical record, and provide appropriate signatures and dating of such entries, including those made in electronic records.

§  Manage the responsibility of meeting face-to-face with the Qualified Professional (QP) Care Managers Staff to review the CA and Treatment plan of the consumer for the development of an action plan to meet the consumer’s needs.

 §  Delivers interventions to support the consumer in the development of various skill building activities, including daily and community living, socialization, and adaptation skills; Development of leisure time interests and activities; Symptoms management; Wellness and substance abuse education; Employment readiness; Behavior and anger management.

§  Provide information pertinent to linkage or integration of multiple services, as they are needed or being received by the consumer, whether between programs, within the area program, or from other outside agencies

§  Perform crisis intervention and  Develop a consumer discharge/transfer plan to ensure movement to the appropriate levels of care. 

§  Documents progress notes for both clinical and billing purposes.

§  Prepare progress reports and notes to reflect the consumer’s behavior/symptoms and/or disabilities.

§  Provide direct face-to-face contacts and services to consumers to ensure that services are reimbursable.

§  Complete progress notes, which are due at the end of the business day after services are rendered to the consumers.

§  Submits consumer’s progress notes should be Medicaid billable; failure to do so will result in non-payment for DSP even though services have been rendered to the consumer.

§  Reports to the Program Manager/designee

 

Minimum Requirements: Must have a master’s degree in a behavioral health-related field including, but not limited to, psychology, nursing, counseling, or social work with two years of addiction treatment experience. Must also possess board certification and licensure as a Substance Abuse Counselor in the state of New Jersey.

·   Drug Screen required initially, at random and for cause.

·   All employees shall submit to drug testing initial, random, and for cause.

·  Submission of a completed application to have your name checked against the Child Abuse Registry Information (CARI).
