The Team Coordinator processes all required clinical documentation to facilitate total, accurate, and timely patient information in the patient medical record.  Responsible for direct care scheduling information, monitoring tracking systems, and visit verification to facilitate billing.  Provides all other clerical support activities for assigned team.

Essential Job Functions/Responsibilities

  • Interacts with the patient or patient’s representative to coordinate the SOC visit.
  • Proactively plans and assigns patients to clinical staff.
  • Performs visual check of forms for discrepancies.
  • Processes discharge documentation in a timely manner to the Medical Record Department.
  • Prints other daily, monthly reports and edits reports as required.
  • Distributes reports to appropriate staff as directed.
  • Maintains assigned logs with accurate/up-to-date information.
  • Participates in quality management activities as assigned.
  • Maintains and updates weekly schedules for interdisciplinary team staff.
  • Notifies the Clinical Team Manager of discrepancies between visits ordered, scheduled, and documented.
  • Verifies visits provided to visits ordered and scheduled using the clinical notes and the schedule.
  • Notifies Clinical Team Manager of inability to cover requests for service.
  • Receives and relays messages for assigned interdisciplinary team staff.
  • Prepares schedules for weekend and on-call coverage.
  • Orders medical supplies as needed.
  • Participates in an on-call rotation for weekend coverage and answering phones after hours.
  • Supports the Agency’s customer service program by promoting and maintaining a responsive, knowledgeable, and accessible approach to all customer contacts.
  • Performs other activities as assigned by the Clinical Team Manager.
