Job Description
We seek a highly qualified Licensed Social Worker to join our team in providing exceptional mental health services. As the designated psychotherapist for assigned facilities, you will conduct comprehensive patient assessments and diagnose and treat a range of cognitive, emotional, behavioral, addictive, and developmental disorders and disabilities. Your expertise, advanced clinical training, and specialized skills will contribute to the well-being of our diverse patient population.
Key Responsibilities:
- Conduct thorough evaluations of patients within nursing homes, both existing and prospective, based on their specific needs, limitations, and desires.
- Maintain a caseload of clients experiencing common mood disorders, substance abuse, and life adjustment challenges.
- Provide evidence-based psychotherapy to clients, adhering to professional standards.
- Develop and implement assessments and treatment plans for clients.
- Participate in team meetings and case consultations as required.
- Maintain accurate and up-to-date client records and documentation in our Electronic Medical Records system.
- Ensure compliance with all relevant laws, regulations, and professional standards.
Additional Details:
- Schedules are flexible, with the option for full-time Monday-Friday or part-time work.
- No case management or discharge planning responsibilities.
- Collaborate with our billing and facility liaison team for caseload management.
- Malpractice insurance and tail coverage are provided by Nexum HC for $1MM/$3MM coverage if not already covered.
Location:
- Pontiac, IL, Gibson, IL, Bloomington, IL area.
- Travel by car to nursing home facilities as needed is required.
Qualifications:
- Master's degree in social work.
- Proficiency in using and learning Electronic Medical Records (EMR) systems.
- Strong communication skills with the ability to provide understandable support and plans to residents and their families.
- Patience, empathy, and problem-solving skills when interacting with patients and their families.
- Ability to conduct psychosocial assessments and develop and implement care plans.
- Bilingual proficiency is a plus but not required.
Salary and Benefits:
- Annual salary: $60,000 for a full-time position.
- 12 days of paid time off (PTO).

Education:
- LSW or MSW level of education.
- Clinical licensure is not required.
