The duties and requirements of the position are to be carried out in a manner that is consistent with the mission, values and operating principles of the Agency.
Assess and develops treatment plan for all new clients for the Children and Families Component.
Applies evidence-based knowledge and acquired skills to assess, diagnose and treat clients with behavioral health problems in an outpatient medical setting.
Completes corresponding assessment and admission clinical documentation in compliance with governing law, regulations, accreditation and Agency requirements.
Adapts interventions to each client’s medical necessity, the work flow and pace of the program, including rapid assessment and brief treatment.
Works collaboratively with the primary care and behavioral health team to ensure that ongoing liaison occurs with clients, medical providers, and families.
Completes documentation (including but not limited to Master Treatment Plan, Treatment Plan Reviews, ASAMs, MHOMF, CFARS/FARS, Discharge Summaries, as applicable) for assigned cases in accordance with timeframes established by licensure, regulations, contracts, Agency policies and procedures, and/or third party payers.
Completes and submits timely prior authorization requests and concurrent authorization requests to the Utilization Management Department to ensure services provided are authorized and reimbursable.
Documents all services in the Electronic Health Record within the defined timeframes.
Completes documentation to demonstrate medical necessity and fidelity to the Agency’s adopted Evidenced Based Practices (EBP’s).
Participates in clinical supervision and team staff meetings as scheduled.
Acts as a clinical leader and resource to impact program development, implement plans, and improve quality of care.
Maintains knowledge of the Agency’s policies and procedures, and program standard operating procedures (SOP’s).
Maintains confidentiality in accordance with HIPAA Privacy Rule, HITECH Omnibus Rule, and 42 CFR.
Completes all required trainings within the timeframe specified by the Agency’s policies and training curriculum.
Provides services in a professional and ethical manner which supports recovery.
Meets the target productivity of 80%. (Minimum of 32 direct service hours per week).

EDUCATION AND/OR EXPERIENCE:
Licensed Clinical Social Worker with a minimum of one year experience in the field.
Ability to read, write and speak in English and Spanish (or other languages) helpful. Must be able to read and comprehend simple instructions, short correspondence, and memorandums. Ability to effectively communicate information both orally and written on one-to-one and small group situations to clients and other employees.
Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
Must be organized and able to schedule work load appropriately without hindrance to client appointments etc.
Must be able to work independently with competent decision making using established criteria and guidelines.
OTHER SKILLS AND ABILITIES:
Computer Skills: ability to work with word processing software, spreadsheet software, presentation software, internet software, and electronic health record.
