Employment Specialist/Job Coach Job Description

Compensation: $15.00 hour starting, $16.00 after one year. Bilingual English/Spanish Fluency additional $1.00/per hour

BASIC FUNCTIONS: To seek, develop and create appropriate competitive employment opportunities for LD/DD individuals and other assigned referrals and to maintain employment support.

Qualifications:
Preferred: Bachelor�s Degree in Social Work, Psychology, Rehabilitation Counseling, Special Education or other closely related field, PLUS one year full-time direct service experience in human services.

Minimum: High School Diploma or equivalency PLUS two years full-time direct service experience in human services.

Background check required, personal vehicle and adequate auto insurance coverage required for position.

RESPONSIBILITIES:

Assess individual�s skills, needs and interests to determine appropriate services.

Create a service plan in cooperation with the individual designed to meet individuals needs and goal(s) and to determine appropriate placement in vocational program.

Meet with individual for vocational counseling and other activities. Meet with individual to develop a professional job search plan and to complete the job readiness activities.

Meet with individual to seek, develop, and create appropriate competitive employment opportunities through a variety of means, including but not limited to, cold calling, newspaper ads, DOL job bank, relevant internal and external committees that increase potential employment opportunities, etc., and follow up on job leads for those who are job ready.

Provide intensive job coaching services, with task analysis as required, at the individuals� job placement site until stabilized.

Document all activities on required department forms within published timelines and provide transitional paperwork, as needed, for follow along services.

Complete necessary reporting documents (time sheets, mileage, and weekly schedules, case notes, billing forms, and incident reports.)
Attend all staff trainings and meetings as scheduled.

Provide support to other vocational staff in order to meet the individual�s needs in a timely and effective manner.

Promote supported employment in the community through outreach activities as assigned.
Perform other duties as assigned.
