RE-ENTRY CASE MANAGER/NAVIGATOR

Organizational Background: Jobs of Hope (JOH) was founded in 2013 for the purpose of providing support for men seeking tore-enter the community and obtain a better quality of life through education, support, housing and employment. The JOH Re-Entry Program serves all of Weld County and is dedicated to provide a targeted and supportive program to help men leave behind years of legal problems, gang affiliation, and substance abuse. Our goal is to be a bridge for individuals to re-connect with their family and community and vice-versa, to help our community engage with this population and share our vision of seeing their potential in becoming positive and productive members of our community. The purpose of JOH is to change the family tree of men who have found themselves justice-involved and are wishing to make a change in their lives and the lives of their children, partners and extended family, and to be a �bridge� for integrating individuals and their families back into the community.

General Description of Duties: The Full Time (40 hours/week) Re-Entry Case Manager/Navigator provides services in a fully integrated, holistic and collaborative manner using a team-treatment approach with community partnering agencies that ensures the coordination of all client services. This integrated model aims to facilitate clear communication amongst all providers and eliminates duplications of efforts or conflicting approaches to care and ensure wrap around support. Communicates directly with clients to obtain historical information on financial status, employment, housing, family supports, legal issues and other potential client needs resulting in the development of a client focused holistic plan. The Re-Entry Case Manager/Navigator will provide a supportive environment to help individuals meet their goals toward self-sufficiency, teach skills to work toward the individual�s self-identified life goals (social, educational, employment, housing, and benefits,), maintain frequent communication with the Intake/Outreach Specialist and Executive Director providing updates on the progress being made toward reaching and/or meeting goals. This position will provide advanced one-on-one assistance to clients of Jobs of Hope regarding access to program services and an extensive coordination with partnering community agencies to ensure clients are successfully connected to resources that will help improve their current situation.

Supervision Received: Direct supervision provided by the Executive Director.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
?Provides a clear understanding of the commitment/expectations of the program to each participant.
?Provides a comprehensive initial appointment assessment to identify strengths and barriers to successful outcomes through interviewing.
?Assists clients in the development of a client focused holistic plan for success. Provides supportive services per program guidelines to assist clients in reaching their established goals.
?Establishes with each client a schedule for ongoing appointments and check ins to meet their goals.
?Establish collaboration, communication and coordination among all current and future community partners� involved in the implementation of the client�s individualized plan.
?Assists clients in scheduling appointments, follow-up and referrals.
?Attends client appointments with community resources as needed.
?Assists with and provides transportation to and from appointments on an as needed basis.
?Serves as a client liaison to communicate client needs and gaps in services.
?Attend organizational and community based meetings with community partners as required to ensure
appropriate service delivery.
?Create and manage client files and documentation which maintains accurate and up-to-date case
records; documents client contacts and progress to ensure program compliance and progress of goals
?Case staffing�s and reviews with supervisor on a regular basis.
?Conduct crisis intervention and conflict resolution as needed and consult with Executive Director on
serious conflicts that can�t be mediated.
?Conduct exit evaluations with clients to measure outcomes achieved.

Other Duties and Responsibilities
?In coordination with other staff, provide in-person coverage at the JOH office during office hours.
?Flexibility to attend various inter-agency meetings or program events in the evenings or weekends as
assigned by Executive Director.
?Attend professional development and community trainings as approved by Executive Director.
?Assists in the development and implementation of program policies and procedures.
?Coordinate responsibilities with other staff members to insure an integrated team approach to all
programs and day-to-day operations of JOH.
?Any other tasks or duties as assigned.

MINIMUM QUALIFICATIONS:
Preference for an individual who has the strength, experience and passion to work with adult males having significant life challenges, such as; incarceration, gang affiliation, substance abuse, mental health and legal issues.

?Knowledge of the basic theories and practices of social work, interviewing and assessment techniques, and professional reports and documents preparation.
?Skills in establishing and maintaining effective working relationships with clients, other employees, organizations, and the public are required.
?Flexible and ability to multi-task. Basic knowledge of computers to input information, maintains client and program data, and creates communications.
?Knowledge of the service population�s cultural and socioeconomic characteristics and the appropriate techniques and practices for the client population.
?Ability to interact effectively as a team member and independently with Agency staff and with a diverse client base.
?Demonstrated qualities of empathy, assertiveness and organization, a high degree of integrity and responsibility.
?Ability to read, understand and follow oral and written instruction.
?Ability to work independently in a small office environment which includes office cleanup and maintenance.
?Timely responsiveness to emails, calls and client requests.
?Maintain confidentiality of client records and communications.
?Reliable transportation and provide proof of insurance.
?Ability to enter data in computer programs and apply new computers skills on a regular basis.
?Proficient in the use of a computer and Microsoft Office programs such as Word and Excel.
?Bilingual preferred

Education and Formal Training:
?Successful completions of a Bachelor�s Degree in Social Work, Human Services, Criminal Justice or related field.
OR
?Equivalent combination of education, training and experience which provides the required skills, knowledge and ability for the position may be considered in lieu of a degree.
?Experience serving vulnerable populations, including ex-offenders and homeless preferred.

Licensure/Certification:
Valid Colorado Driver�s License

Working Environment Physical Requirements:
Frequent contact with client and public by phone or in person
?Requires sitting for long periods of time (60% of time).
?Some bending, stretching, reaching, crouching, pulling, lifting, carrying, and handling required.
?Manual dexterity required for use of calculator, computer keyboard, photocopier, telephone, calculator, and other office equipment.
?Climbing up and down stairs
?Requires a normal range of hearing and eyesight to communicate with clients and public.
