Job description
Access TLC Home Health Care is seeking a Full-time Care Coordinator for our busy ECM program.

We support patients by arranging for community-based and other services that allow recipients to experience an enhanced and enriched quality of life. The right candidate will be eager to learn, compassionate, kind and a genuine team player. Bi-Lingual is a plus! Healthcare background is preferred.

Requirements include:

Must have either a Bachelor or Master Degree in Social Work, Psychology, Counseling, Rehabilitation, Gerontology, Sociology, Health or Human Services or related field; plus one year of related work experience preferred.
Must have a clean California Driver�s License with a vehicle that is insured in accordance with state/or organization requirements and is in good working order
Job Type: Full-time

Schedule: Monday through Friday 8:30am-5:30pm

Rate of pay: $20.00/hour.

Position Responsibilities:

Conducting assessments, developing care plans, arranging for, and monitoring service delivery, maintaining progress notes and case records, conducting quality assurance reviews, and collecting data.
Makes monthly visits to patients to establish ongoing eligibility for the ALW program, to monitor the safety and services identified in the Individual Service Plan and address the concerns of the patient and the facility.
