Job Title: Registered Nurse – Case Manager
Location: Anchorage, AK
Type: Contract (Travel Only)
Shift: Day Shift | 07:30 AM – 04:00 PM
Schedule: 5x8 Hours
Duration: 13 Weeks
Start Date: 05/18/2026

Position Overview:
The RN Case Manager coordinates patient care across the continuum, ensuring effective resource utilization and safe, timely discharge planning in an acute care setting.

Responsibilities:
• Coordinate care plans with multidisciplinary teams
• Perform utilization review and manage resource allocation
• Develop and implement discharge planning
• Communicate with patients, families, and providers
• Ensure continuity of care from admission to discharge
• Document in Epic EMR
• Identify and resolve care/discharge barriers
• Support quality and compliance initiatives

Qualifications:
• Active Alaska RN License – Required
• Minimum 1+ year Case Management experience – Required
• Acute care experience – Required
• Epic EMR experience – Required
• BLS Certification – Required
• Strong clinical, communication, and organizational skills

Preferred:
• ACM Certification
• Experience in Magnet / Level II Trauma facilities
• Utilization review or discharge planning experience

Additional Details:
• Local candidates not accepted (must reside 50+ miles away)
• Weekend requirement: Every other weekend / 4 days per 4 weeks
