Responsibilities

• Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, feeding, and meal preparation.
• Provide personal care and behavioral support for individuals with developmental and intellectual disabilities.
• Support mobility, transfers, and safe ambulation as needed.
• Monitor client health, observe and report behavioral or medical changes to the RN or supervisor.
• Follow individualized care plans while maintaining client rights and confidentiality (HIPAA compliant).
• Engage clients in meaningful activities, community outings, and social interactions to promote mental and emotional well-being.
• Maintain a clean, safe, and organized home environment.
• Communicate and collaborate with families, case managers, social workers, and healthcare professionals.
• Document care and communication clearly and accurately.

Skills & Experience

• First Aid & CPR certification (provided if you don't have it)
• Experience in group homes, supported living, or assisted living highly desirable
• Strong observation skills for monitoring behavioral or health changes
• Experience working with individuals with autism, Down syndrome, or other developmental disabilities
• Ability to lift, transfer, and assist with mobility (up to 50 lbs)
• Cooking ability for simple and dietary-specific meals
• Home care, home health, or hospice experience is a plus
• Excellent communication and teamwork skills

Qualifications

• High school diploma or equivalent
• At least 6 months of experience supporting individuals with developmental disabilities
• Negative TB test or clear chest X-ray
• Pass pre-employment drug screening
• Pass physical examination
• LiveScan background check
• Valid driver's license and reliable transportation preferred
