Our client is seeking an experienced Director of Case Management to lead and oversee case management, discharge planning, and utilization management functions within an acute‑care hospital setting. This leader will ensure regulatory compliance, support efficient patient throughput, and drive high‑quality outcomes across the continuum of care. The ideal candidate brings strong clinical judgment, operational insight, and a commitment to patient‑centered, collaborative care.

Requirements:

  • Manage departmental budgets and support internal and external audits.
  • Foster a supportive, growth‑oriented environment for case management staff.
  • Review and enhance processes related to documentation, throughput, and patient experience.
  • Provide strategic and operational leadership for case management, utilization review, and discharge planning.
  • Monitor patient flow, care coordination, and resource utilization to support timely, safe transitions of care.
  • Ensure compliance with Medicare, CMS, and all applicable regulatory and accreditation requirements.
  • Develop departmental goals, policies, workflows, and performance standards aligned with organizational priorities.
  • Integrate evidence‑based practices into case management processes and quality initiatives.
  • Collaborate with multidisciplinary teams to reduce avoidable delays and optimize care transitions.
  • Oversee staffing, hiring, onboarding, scheduling, evaluations, and ongoing professional development.

Qualifications (Must-Haves)

  • BLS certification.
  • Current RN license (Iowa or multistate compact)
  • Graduate of an accredited Registered Nursing program.
  • Excellent communication, organizational, and problem‑solving skills.
  • Demonstrated leadership experience in a hospital or healthcare environment.
  • Ability to work collaboratively across disciplines and lead teams through change.
  • Bachelor's degree required; Master's degree preferred (Nursing, Social Work, Healthcare Administration, or related field)
  • Minimum 2 years of case management experience in utilization management, discharge planning, or related cost/quality management programs.
  • Strong knowledge of Medicare regulations, CMS requirements, and hospital compliance standards.

Preferred Skills:

  • Master's degree (MSN, MSW, MHA, MBA, or related field)
  • Experience leading case management in an acute‑care hospital.
  • Familiarity with throughput initiatives, quality metrics, and care‑coordination best practices.
