Job Title: Manager of Case Management

Position Summary:
The Manager of Case Management is responsible for overseeing the daily operations of the Case Management and Utilization Review departments. This role ensures the appropriate utilization of hospital services and resources while maintaining the highest standards of patient care. The Manager will work collaboratively with internal and external stakeholders to support effective patient flow, discharge planning, and compliance with all applicable regulations.
Essential Duties and Responsibilities:
Manage the day-to-day operations of Case Management and Utilization Review functions.
Develop and implement policies, procedures, and workflows to support appropriate utilization and continuity of care.
Monitor compliance with all federal, state, and accreditation requirements related to case management and utilization review.
Coordinate interdisciplinary discharge planning and ensure timely transitions of care.
Collaborate with clinical teams, physicians, and external agencies to support optimal patient outcomes.
Maintain confidentiality of patient information in accordance with HIPAA regulations.
Perform other related duties as assigned.
Minimum Qualifications:
Active Registered Nurse (RN) license in the State of California – Required
Minimum of five (5) years of case management experience – Preferred
Knowledge of state and federal regulations and accreditation standards – Required
Bachelor’s degree in Nursing or a related field – Preferred
Experience in a leadership or supervisory role – Preferred
Physical Requirements:
Prolonged periods of sitting, walking, or standing.
Ability to lift and move up to 50 pounds.
Frequent bending, reaching, kneeling, and crouching.
Manual dexterity, visual and auditory acuity required to perform essential job duties.
Must be able to respond quickly in emergency situations.
Work Environment:
Hospital setting with exposure to varying and unpredictable situations.
May be exposed to patients with illnesses or injuries.
May be required to work irregular hours, including weekends or holidays.
