We are seeking a skilled and compassionate Primary Care Registered Nurse (RN) to provide high-quality, patient-centered care in a multidisciplinary outpatient environment. In this role, the RN will support patients by combining clinical expertise, care coordination, patient education, and procedural assistance.
The ideal candidate is confident in clinical decision-making, possesses strong patient assessment skills, and is able to collaborate effectively with physicians and healthcare teams to promote wellness and manage chronic conditions. This role requires the ability to respond to urgent clinical situations and coordinate care across multiple service lines while maintaining a strong focus on compassionate patient care.
Schedule
- Five 8-hour shifts per week
- Daytime outpatient schedule
Key Responsibilities
Patient Assessment and Care
- Conduct comprehensive patient assessments including health history, symptom evaluation, and physical examinations.
- Monitor and manage acute and chronic health conditions.
- Collaborate with physicians and healthcare providers to develop and implement treatment plans.
- Provide clinical support during patient visits and procedures.
Care Coordination
- Coordinate patient care services including specialist referrals, diagnostic testing, and follow-up appointments.
- Facilitate smooth transitions between care settings.
- Communicate effectively with interdisciplinary healthcare teams to ensure continuity of care.
Documentation and Record Keeping
- Maintain accurate and detailed patient documentation including medical history, treatments, and outcomes.
- Ensure documentation complies with regulatory standards and organizational policies.
- Utilize electronic health record systems to track and manage patient care.
Patient Advocacy
- Advocate for patient needs, preferences, and rights during care planning and decision-making.
- Act as a liaison between patients, families, and healthcare providers.
- Provide education to patients and families regarding treatment plans and disease management.
Emergency Response
- Assess and respond to urgent or emergent clinical situations within the scope of practice.
- Initiate emergency protocols when necessary.
- Assist in escalating care and coordinating emergency services when needed.
Required Qualifications
Licensure
- Current and unrestricted Registered Nurse (RN) license in the United States
Certification
- Current Basic Life Support (BLS) certification through the American Heart Association
Experience
- Minimum 4 years of current nursing experience
- At least 2 years of inpatient nursing experience involving care coordination
- At least 2 years of outpatient nursing experience
- Primary care RN experience required
Education
- Associate Degree in Nursing (ASN) required
- Bachelor of Science in Nursing (BSN) preferred
- Degree must be from a program accredited by ACEN or CCNE
