Job Summary:

We are seeking an experienced and compassionate MDS Coordinator to join our team at Carlton Shores Health and Rehabilitation. The ideal candidate will be responsible for coordinating the assessment, care planning, and care delivery processes, ensuring that our patients receive the highest level of care and service.

Responsibilities:

• Coordinate the MDS process, including reviewing medical records, conducting patient assessments, and developing individualized care plans

• Collaborate with healthcare professionals to ensure that patient needs are met and care plans are updated regularly

• Monitor patient progress, identify areas for improvement, and implement changes as necessary

• Communicate effectively with patients, families, and healthcare team members to ensure seamless transitions and continuity of care

• Participate in quality improvement initiatives and educate staff on MDS principles and best practices

• Maintain accurate and complete records, including patient assessments, care plans, and progress notes

Requirements:

• 3 years of experience in a similar role or related field

• Strong knowledge of MDS principles and regulations

• Excellent communication, organizational, and interpersonal skills

• Ability to work effectively in a fast-paced environment and prioritize tasks accordingly

• Strong attention to detail and ability to maintain accurate records
