Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to assist them to live as independently as possible in a community-based setting. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking a qualified candidate interested in joining our Residential Service Group Home team at our location New Castle County, Delaware.

Work Schedule: Total 40 hours per week, M-F and 8 hours shifts

Hire requirements for this opportunity include attending new-hire orientation during the first five (5) workdays (Mon–Fri), at our Philadelphia location. Mileage reimbursement is provided.

Some Residential Service Assistant duties include:

  • Facilitate and reinforce skill development in social supports, in areas including food management, self-care preservation/personal hygiene, relapse prevention, medication management, self-care preservation/personal hygiene, money management money, and planned community activities.
  • Complete documentation, including progress notes and service plans, as required by regulations.
  • Establish an on-going dialogue with other members of the treatment team, including program staff, vocational/educational instructors, consultants, members of the medical profession and family members to provide continuous supports.
  • Perform other duties and responsibilities as they relate to the overall enhancement of program service delivery.

Qualifications:

  • High school diploma or GED required 2 years’ experience working in the mental health field or any combination of experience, education or training that would provide the level of knowledge, skill and ability preferred.
  • Very good communication skills and detailed oriented. .
  • Ability to work independently and part of a team.
  • Proficiency in various computer technologies including Electronic Health Record, Outlook, MS Word and Excel.
  • Valid Driver’s License with acceptable driving record.

Scope:

  • Requires the ability to lift, carry, fold, kneel, stack, stoop and bend which may include direct intervention with
    participants
  • Frequent talking and listening.
  • Possess manual dexterity and fine motor skills.
  • Requires crisis intervention in emergency situations.
  • May be required to report to work during emergencies including inclement weather.
