This job helps people connect with their community and get the support they need.

• Work with people, families, and community partners.
• Help plan community activities and events.
• Build partnerships and share information about services.
• Connect people to resources and opportunities in the community.
• Keep records and work with staff to support program goals.
• Follow safety, privacy, and DBHDS rules.

• Experience in human services, behavioral health, or disability services is preferred.
• Good communication, organization, and people skills.
• A valid driver’s license will be needed.
• Must be able to complete required background checks and training.

• Understand person-centered support, privacy, and community inclusion.
• Communicate well and work with different people.
• Stay organized, keep records, and handle several tasks.
• Use good judgment and work well alone or with a team.

This job may include local driving, flexible hours, and work in both office and community settings. Staff must follow agency policy, DBHDS rules, and person-centered practices.
