Job Description Overview

We are currently seeking an Intake Data Analyst to assist. In this position, the ideal candidate would conduct initial intake assessments, case management, counseling, and care coordination for the clients and families. You will also process all referrals for clinical grants and wrap-around services, both external and internal.

Duties and Responsibilities

· Conduct initial intake assessments to determine eligibility of prospective clients, ensuring a welcoming, respectful, and client-centered experience.

· Coordinate and maintain all program records, supporting accurate data collection for evidence-based services, including facilitation of pre- and post-surveys.

· Gather and document medical histories and relevant personal information with sensitivity, professionalism, and respect for client confidentiality.

· Facilitate and coordinate internal and external referrals to connect clients with appropriate services and community resources.

· Maintain and update client records to ensure accuracy, completeness, and continuity of care.

· Review documentation for accuracy and compliance with program and funding requirements.

· Prepare new client intake paperwork and complete required assessments in a timely and supportive manner.

· Assist with billing processes as needed, ensuring proper documentation and alignment with program guidelines.

· Provide support with front desk and receptionist duties, including warmly greeting clients, responding to inquiries, and helping create a positive, accessible, and inclusive environment.

· Train sub-grantees and staff on data reporting procedures, promoting consistency, accountability, and program effectiveness.

· Develop and maintain tracking systems to monitor client engagement, program outcomes, and reporting requirements.

· Perform additional duties as assigned to support the mission, goals, and daily operations of the organization.

Requirements & Qualifications

· High school diploma or GED certificate

· Associate or Bachelor’s Degree (preferred)

· Client scheduling experience

· Experience with health insurance

· Computer and data entry skills

· Bilingual communication skills is a plus

· Applicants should have some experience in an administrative role in a mental health setting, and possess excellent data entry skills.
