An innovative Advanced Primary Care practice in Munster, Indiana is seeking two Nurse Practitioners or Physician Assistants to join their growing team. This is a permanent placement opportunity with schedule flexibility.

The practice takes a tech-enabled, data-driven approach to delivering high-touch primary care and chronic disease management across a collaborative, multi-disciplinary team that includes Family Medicine, Internal Medicine, Geriatrics, Mental Health, and Obesity Medicine.

Requirements
