Support Team Trainer (Part-Time/Full-Time)
20.00 per hour
This job description provides an overview of the requirements, responsibilities, and expectations for the Support Team Trainer position. It is not an employment contract. Employment with Service to Achieve Growth and Empower, Inc. dba SAGE Services/dba Coastline Services is at-will, meaning either party may end the relationship at any time, with or without cause or notice. Required Qualifications: High School diploma/GED. Relevant paid or unpaid experience or education (supported/independent living, day program, behavioral services, direct care, developmental disabilities). Ability to lift up to 25 lbs and complete sustained physical tasks (standing, walking, bending, stairs, adaptive equipment use) for at least 45 minutes.
Conditions of Employment: After acceptance of a conditional offer, employees must provide and maintain: valid I-9, acceptable DMV record, valid CA Driver’s License, active auto insurance, reliable transportation, required trainings (First Aid/CPR, workplace violence, sexual harassment, etc.), negative drug/alcohol screen per CA law, DOJ clearance, and the ability to meet all job duties. Employees must maintain confidentiality, professionalism, follow policies, deadlines, attend meetings/trainings, and meet scheduling needs. Failure to meet any condition may result in disqualification or termination. All employees must pass a DOJ background check per Tri-Counties Regional Center (TCRC) and Department of Developmental Services (DDS) requirements. Any findings that do not meet criteria will result in immediate disqualification or separation. Honesty is required. Any falsification, omission, or misrepresentation on applications or documents will result in corrective action or termination.
Responsibilities May Include but Are Not Limited To:
Mission and Scope: Follow SAGE’s mission, values, policies, and Personal Attendant regulations under California Industrial Welfare Commission Order 15-2001. SAGE does not provide medical, nursing, home-health, or caregiving services. Employees must remain within scope.
Level of Support: Provide individualized support in homes and the community, promoting participation, skill-building, independence, and meeting goals outlined in the IPP. Support may include hands-on assistance when individuals cannot participate fully. All services use a person-centered approach.
Medication Support: Work with the team to provide medication reminders, support, refills, pharmacy pick-up, documentation, communication, and reporting. Accuracy is mandatory. All medication errors are reported to TCRC/DDS through an SIR.
Appointments/Schedules: Support individuals in creating meaningful schedules, locating activities, attending events/classes on time, and building community connections. Assist with finding physicians, scheduling, attending, completing follow-ups, and reporting changes.
Engagement: Employees must remain fully engaged, present, and attentive. Personal distractions are prohibited unless approved. Maintain line-of-sight when required. Employees cannot leave shifts early, run personal errands, take unauthorized breaks, or disengage. Staff must wait for scheduled relief when applicable. All interactions must support dignity, independence, and goals.
Money Management: Support budgeting, bill paying, banking, and mail. Borrowing, lending, or exchanging goods, services, or money with persons served or families is prohibited without written management approval.
Health and Wellness: Support personal hygiene/grooming, transfers with adaptive equipment, grocery shopping, meal planning, cooking, cleaning, safety skills, laundry, technology, transportation, mobility training, and activity participation. Follow behavioral plans and collaborate with clinical teams regarding behavioral/medical challenges.
Hands-Off Approach: SAGE strictly prohibits physical or chemical restraints. Employees must use proactive and positive behavior support strategies and approved safety techniques only.
Crisis Response: Employees must be prepared to respond to medical, behavioral, or environmental emergencies. Staff must act calmly, follow emergency protocols, report immediately, and comply with all essential worker expectations.
Safe Work Environment: Follow health and safety rules including adaptive equipment use, body mechanics, infection control, hazardous material handling, and incident reporting. Attend all required safety trainings and apply practices consistently.
Overnight Support: Typically 10pm–8am. Staff must remain awake when individuals are awake and may sleep when individuals are asleep and do not require support. Overnight duties may mirror daytime tasks.
Communication/Documentation: Maintain positive and effective communication. Check schedules daily, complete documentation by end of shift, ensure accurate timekeeping, collect data, follow directives, maintain confidentiality, complete mandated paperwork, and meet all deadlines.
Confidentiality: Maintain strict confidentiality of all personal, medical, financial, and company information. Unauthorized disclosure is prohibited and subject to corrective action up to termination.
Mandated Reporting: Employees must immediately report suspected abuse, neglect, exploitation, scams, or harm to appropriate authorities (APS, police, TCRC) and internal management within required timelines. Compliance is legally required.
Professionalism: Maintain professional behavior and adhere to code of conduct, dress code, and ethical standards. Zero tolerance exists for abuse, neglect, bullying, harassment, discrimination, retaliation, or misconduct. Always request permission before entering homes or using household items. Maintain respect, dignity, and integrity at all times.
Training: Provide quality training and modeling for new and existing staff. Attend and participate in required trainings, meetings, and events. Collaborate with persons served and their teams, follow directives, communicate effectively, take initiative, accept feedback, and maintain a positive and professional attitude.
Schedule/Expectations: SAGE operates 24/7 throughout Ventura County. Support Team Trainer schedules vary weekly based on person served needs and employee availability. Schedule changes within the week must be followed. Workweek is Monday 12am through Sunday 11:59pm. Overtime after 40 hours is for full-time employees only and requires prior approval. Unauthorized overtime or schedule changes are prohibited. Secondary employment is allowed if it does not conflict with availability, transportation, or job duties. Excessive requests for changes may affect employment status. The role is fully in-person.
Additional Information: Supported Living Services requires flexibility, including last-minute changes. Reliable attendance and punctuality are essential. Repeated tardiness, absences, or failure to adhere to assignments may result in corrective action or termination. Employees must be able to perform all essential job functions, with or without reasonable accommodation. SAGE will engage in the interactive process when needed.
