Position Summary

We are seeking a skilled and caring Physical Therapist (PT) to join our growing home health team. The ideal candidate will be passionate about helping patients regain strength, mobility, and independence in their home environment.

Responsibilities

• Perform patient assessments and develop individualized treatment plans
• Provide in-home therapeutic exercises, mobility training, and education
• Collaborate with nurses, occupational therapists, and physicians to ensure coordinated care
• Document patient progress and visits in accordance with agency and Medicare standards
  
   

Qualifications

• Current California Physical Therapist license (required)
• Minimum 1 year of PT experience (Home Health experience preferred, not required)
• Reliable transportation and valid driver’s license
• Strong communication and organizational skills
• Compassionate, patient-centered approach to care
