Carter Burden Network (CBN) is seeking a compassionate and detail-oriented Case Worker to join the Roosevelt Island Older Adult Center team for a temporary 12-week leave replacement, with the possibility of extension based on program needs. This position plays an essential role in connecting older adults to vital resources, benefits, and community supports that enhance quality of life and foster independence.
The Case Worker will provide individualized assistance, conduct needs assessments, facilitate referrals, and maintain accurate case documentation while working closely with participants, staff, and local community partners. Essential Duties & Responsibilities
Client Support & Case Management
- Provide individualized support to older adults by conducting needs assessments, making appropriate referrals, and assisting with benefit applications.
- Act as a liaison with community agencies and service providers to connect clients to programs that address food insecurity, healthcare access, and overall quality of life.
- Provide information and assistance to clients regarding workshops, events, and social opportunities both on Roosevelt Island and within the broader CBN network.
- Maintain patience, empathy, and professionalism when handling sensitive situations and complex client needs.
Outreach & Community Engagement
- Conduct outreach and build relationships with community partners and stakeholders on Roosevelt Island.
- Research and distribute information about community resources and upcoming programs.
- Support CBN events, workshops, and wellness initiatives to increase participation and awareness.
Documentation & Data Management
- Accurately record daily client contacts, referrals, and service data in CBN’s case management system.
- Run monthly reports and maintain compliance with internal and NYC Aging documentation standards.
- Assist with reporting and data collection as needed for audits or program evaluation.
Collaboration & Program Support
- Participate in staff meetings, agency events, in-service trainings, and conferences as assigned.
- Work closely with the Program Director to ensure files and records are organized and accessible.
- Engage professionally and collaboratively with colleagues across CBN programs and partner organizations.
Additional Duties
- Assess clients for emergency and stipendiary grant eligibility.
- Perform other responsibilities as needed to meet program and client needs.

Qualifications
- Bachelor’s degree with 1–3 years of relevant experience, preferably with older adults; background/degree in Social Work, Human Services, Psychology, Gerontology, or related field preferred.
- Verbal and written bilingual proficiency in English required; bilingual in Spanish or other language a plus.
- Experience with client advocacy, benefits navigation, and resource coordination.
The Case Worker role offers a competitive salary range between $26.72–$28.37 per hour (annualized equivalent $48,630–$51,633 per year) in full compliance with the NYC Pay Transparency Act, with final placement based on experience, skills, and internal equity. This is a full-time, non-exempt position (35 hours per week) with a one-hour unpaid lunch break. The schedule is Monday through Friday, with occasional evening or weekend hours required with advance notice. This is a benefits-eligible position.
