Position Overview:
We are seeking dedicated and compassionate Physical Therapists and Physical Therapist Assistants to join our growing home health team. In this role, you’ll help patients regain strength, mobility, and independence through individualized therapy delivered in the comfort of their homes. You’ll have the freedom to work independently while collaborating with a supportive interdisciplinary team to design and implement personalized care plans. Key Responsibilities:
-Provide initial and ongoing comprehensive assessments of each patient’s needs, including completion of OASIS assessments.
-Develop and implement the interdisciplinary plan of care as it pertains to physical therapy.
-Assist the physician in evaluating patients by applying diagnostic and prognostic tests of muscle, nerve, joint, and functional abilities.
-Consult with and educate staff regarding the established therapy plan.
-Teach, supervise, and counsel the patient and family members in the physical therapy program to promote independence and safety.
-Instruct patients in the proper care and use of assistive devices such as wheelchairs, braces, crutches, canes, prosthetics, and orthotics.
-Recommend and arrange for equipment necessary to carry out the therapy plan.
-Coordinate outpatient services when certain procedures cannot be provided in the home.
-Document assessments, progress notes, and evaluations in accordance with agency policy and regulatory timelines (e.g., notes completed on date of visit and filed within 48 hours).
-Supervise Physical Therapist Assistants (PTAs) and Home Health Aides (HHAs) per agency policy, including direct observation at required intervals.
-Monitor and report patient reactions to treatment, changes in condition, and progress toward goals to the physician and care team.
-Communicate regularly with the physician (at least every 30 days or when changes occur) to update on patient status and obtain new or revised orders.
-Collaborate with interdisciplinary team members, including HHAs, and provide instruction on therapy-related activities as appropriate.
-Participate in case conferences, staff meetings, and in-service training to maintain coordination and professional growth.
-Contribute to discharge planning and complete a Physical Therapy Discharge Summary when therapy goals are met or services are discontinued.
Admission Support
-Assist in the admission process by obtaining a relevant medical history, evaluating the patient and home environment, and identifying the level of family and community support available.
-Ensure physician orders are accurate and appropriate, and discuss necessary changes or clarifications as needed.
Qualifications:
-Successful completion of an accredited Physical Therapist (PT) or Physical Therapist Assistant (PTA) education program approved by one of the following:
--The Commission on Accreditation in Physical Therapy Education (CAPTE), or
--A successor organization of CAPTE, or
--An international education program determined to be substantially equivalent to U.S. entry-level PT or PTA education by a credentialing organization approved by the American Physical Therapy Association (APTA) or one recognized under 8 CFR 212.15(e).
-Successful completion of a state-approved licensure examination for Physical Therapists or Physical Therapist Assistants.
-Active Massachusetts PT or PTA license/registration (required).
-Minimum of one year of experience in a clinical, institutional, or home health setting (home health experience preferred).
-Must pass a criminal background check.
-Must hold a current CPR certification that includes an in-person skills assessment (online-only certifications not accepted).
- Strong knowledge of physical therapy principles, techniques, and equipment.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a collaborative healthcare team.

- Valid driver’s license and reliable transportation to visit patient homes.
