Job Summary
We are seeking a compassionate and skilled Home Care Nurse to provide high-quality nursing care to patients in their homes. The ideal candidate will have a strong background in various medical fields, including pediatrics, internal medicine, and outpatient care. As a Home Care Nurse, you will play a crucial role in enhancing the quality of life for patients by delivering personalized care tailored to their individual needs.

Duties

• Conduct comprehensive assessments of patients' health and medical needs in their home environment.
• Develop and implement individualized care plans based on patient assessments and physician recommendations.
• Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
• Monitor patients’ vital signs and health status, documenting changes and communicating with healthcare providers as necessary.
• Provide education and support to patients and their families regarding health management, medication administration, and lifestyle modifications.
• Collaborate with interdisciplinary teams to coordinate care for patients in assisted living or home community settings.
• Perform basic life support as needed during emergencies or health crises.
• Maintain accurate records of patient care activities, including coding for services rendered and laboratory results.

Experience

• A valid nursing license with experience in home community care
• Experience working in hospitals or assisted living facilities is a plus.
• Proficiency in using healthcare systems for documentation and coding purposes is desirable.
• Strong communication skills to effectively interact with patients, families, and healthcare professionals.
