Position Overview: The Intake Case Manager (CM) has the responsibility for developing, authorizing, coordinating, and implementing community-based services to older adults and people with disabilities.
Position Responsibilities: Receive assignments from Home Care Triage for potential consumers. Conduct Initial Assessment to determine eligibility for GLSS programs. Develop appropriate service plan based EOEA-mandated assessment tool, and process all paperwork/computer work. Make referrals for Home Care and/or other community services according to the Home Care referral protocol. Coordinate with GLSS Referral Specialist and service providers to ensure timely and non-duplicative service delivery. Contribute to an Intake Team identity and role based on consumer engagement, outreach, motivational interviewing, multi-functional assessment, and consumer driven care.

Hours 40 hours per week, Monday through Friday
