Physical Therapist
Care For Less is seeking a highly motivated and energetic Physical Therapist to join our compassionate team. The ideal candidate is committed to supporting a positive, professional, and patient-centered environment. This position involves providing in-home therapy services and helping patients achieve optimal physical function and mobility.
Key Responsibilities:Perform initial and ongoing assessments of the patient’s physical condition.
Develop and implement personalized care plans with measurable goals.
Deliver skilled physical therapy interventions, exercises, and treatments.
Instruct patients and caregivers on at-home rehabilitation and safety.
Coordinate care with physicians, nurses, and the Care For Less team.
Maintain timely and accurate documentation of patient progress.
Adhere to all clinical and safety standards and regulations.
Requirements:Valid Physical Therapist License in New Jersey (Compact license accepted).
Minimum of 1 year of experience in a Skilled Nursing Facility (SNF).
No more than 30 days of gap in entire work history.
1 professional reference from work history.
Social Security Number (SSN) and Date of Birth (DOB) required for compliance.
Strong communication, organization, and interpersonal skills.
Preferred:Experience in Homecare/Home Health setting.
Bilingual skills are a plus.
