Job Title: Intake Assessor
REPORTS TO: Clinical Director
SUPERVISES: NONE
DUTIES AND RESPONSIBILITIES:
1. Complete intake interviews to determine/assess consumers need for community support services; complete reauthorization of consumers for CMO services.
2. Follows up to determine progress and attainment of treatment goals; makes recommendations regarding modification to services and service delivery; complete reauthorization for ongoing services.
3. Provide case management, family and individual counseling, to consumers and families as appropriate to the position; prepare treatment plans, bio- psychosocial assessments; provides therapeutic crisis intervention and emergency services as needed.
4. Gather information about consumers through interviews, collateral contact, psychological tests, and/or observational techniques; evaluates data to identify causes of problems and to determine proper therapeutic approaches and interventions.
5. Facilitate community outreach and marketing to solicit referrals for agency programs.
6. Maintain accurate and timely documentation of case records; modify ISP as needed; and complete other required paperwork as needed.
7. Provide, arrange, and refer for therapeutic interventions as appropriate for consumers in a crisis condition and for those with serious mental health problems.
8. Maintain professional relationship with community agencies and referral resources; remain in frequent contact to inform of intake status.
9. Complete monthly program and statistical reports.
10. Monitor status of all referrals forms and ensure timely completion of assessments.
11. Consults with other therapists and related professional and paraprofessional staff, as appropriate, in the performance of therapeutic and/or casework; refer clients to appropriate service agencies as required.
12. Participates in the review of clinical issues and program policies and procedures.
13. Participant in weekly clinical team meetings.
14. Consult with therapists and related professional and paraprofessional staff, as appropriate, in the performance of therapeutic and/or authorization paperwork; refer clients to appropriate service agencies as required.
15. Perform miscellaneous job-related duties as assigned.
MINIMUM JOB REQUIREMENTS:
Master’s degree in Counseling, Social Work, or related Field; with at least 2 years of professional experience completing bio-social assessments and treatment planning.
Candidate must be licensed or working towards licensure by the GA composite Board.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
Knowledge and understanding of clinical social work and mental health counseling disorders and treatment methods, procedures, and standards.
Knowledge of Microsoft office products (Word, Excel, Access).
Good organizational and interpersonal skills.
Team player and team builder.
Good oral and written communication skills.
Good assessment interviewing skills.
Ability to work independently, multi-task, and meet service objectives efficiently.
Knowledge of proper mental health record keeping and documentation.
Knowledge of crisis intervention techniques.
Knowledge of community mental health.
Knowledge of completing treatment plans, ISP’s, and bio-psychosocial assessments.
Typical Working Conditions:
Most work with this agency is performed with children (under the age of 18 y/o) with mental illness or substance abuse illness. Services are usually provided in the home, school or in other community settings. Evening and Weekend work may be required, depending on the needs of your families. The job involves collaborating and maintaining regular contact with Psychologist, Psychiatrist, DFCS, DJJ, and other community agencies.
