Job description

Job Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide essential nursing care to patients, ensuring their comfort and well-being. The LPN participates in the delivery of health care services and other support services to individuals with developmental disabilities in homes in the community. The candidate works to promote good health, prevention of illness, and support individuals to maximize their independence and become active members of their communities. The LPN works closely with direct support staff and are considered a part of coverage. The candidate also monitors Medication Administration and completes necessary documentation as requested.

Responsibilities

  • Provide exceptional nursing care and oversight by implementing the individuals' health care plan including charting vital signs, application of medical/surgical dressings, and administering medications as prescribed and directed by the primary/specialty physician.
  • Provide direct patient care, including assisting with daily living activities.
  • Conduct assessments of patients' health status and report changes to the supervising nurse.
  • Assist in family planning and education regarding health maintenance.
  • Collaborate with healthcare teams to develop and implement patient care plans.
  • Perform procedures such as spinal taps and assist in the operating room (OR) as needed.
  • Communicate health issues to staff and update the individuals' families/guardians in matters including hospitalizations, rehabilitation support such as physical/occupational therapy.
  • Assist with daily maintenance, personal care and other services of support as outlined in the Individual Service Plan (ISP); Maintain individual records and files.
  • Support individuals with developmental disabilities through tailored care approaches.
  • Maintain accurate patient records and documentation in compliance with healthcare regulations.
  • Lift and transport patients safely, ensuring their comfort during transfers.
  • Order medications and maintain medical supplies, materials and equipment
  • Initiate emergency medical procedures by applying basic first aid techniques and/or CPR and tracheotomy care, and catheterizations;
  • Provide training for direct support staff on use of equipment and treatments.
  • Transport individuals to work, medical appointments, recreational activities and other community engagements as needed.

Qualifications

  • Valid Licensed Practical Nurse (LPN) license in the state of practice.
  • Experience in pediatrics, acute care, or working with individuals with developmental disabilities is preferred.
  • Proficient in medication administration and basic nursing procedures.
  • Strong communication skills to effectively interact with patients, families, and healthcare team members.
  • Ability to lift patients and perform physical tasks associated with nursing care.
  • Compassionate demeanor with a commitment to providing high-quality patient care.
