Job Title: Full-Time Case Manager

Position Summary:
The Haven Men’s Shelter is seeking a dedicated and experienced Full-Time Case Manager to join our team. The Case Manager will be responsible for providing comprehensive case management services to individuals and families, with a focus on rapid re-housing and veterans housing. The ideal candidate will have a strong background in case management, excellent interpersonal skills, and a commitment to supporting our clients in achieving housing stability and self-sufficiency.
Key Responsibilities:
Case Management:
Conduct thorough assessments to identify client needs, strengths, and barriers.
Develop individualized service plans with specific goals and objectives.
Provide ongoing case management services, including crisis intervention, advocacy, and referrals to community resources.
Maintain accurate and up-to-date client records in accordance with agency and funding requirements.
Rapid Re-Housing:
Assist clients in securing stable housing through rapid re-housing initiatives.
Collaborate with landlords and property managers to identify suitable housing options.
Provide financial assistance and support to clients for move-in costs, rental assistance, and other housing-related expenses.
Monitor and evaluate clients' progress towards housing stability and self-sufficiency.
Veterans Housing:
Serve as a primary point of contact for veterans seeking housing assistance.
Develop and implement tailored housing plans for veterans, considering their unique needs and circumstances.
Coordinate with local veterans' services organizations and government agencies to access available resources and benefits.
Advocate for veterans' housing rights and support their integration into the community.
Collaboration and Advocacy:
Work closely with other service providers, community organizations, and stakeholders to ensure a coordinated approach to client services.
Advocate for clients' needs and rights within the community and with service providers.
Participate in community outreach and education efforts to raise awareness of The Haven's services and the needs of our clients.
Professional Development:
Stay current with best practices and emerging trends in case management, rapid re-housing, and veterans housing.
Attend relevant training, workshops, and conferences to enhance professional skills and knowledge.
Participate in regular supervision and team meetings.
Qualifications:
Bachelor's degree in Social Work, Human Services, or a related field
Minimum of 3 years of experience in case management, rapid re-housing, or veterans housing.
Knowledge of local housing resources, veterans' services, and community support systems.
Strong organizational and time management skills.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Proficiency in Microsoft Office Suite and case management software.
Valid driver's license and reliable transportation.
Working Conditions:
Full-time position (40 hours per week) with occasional evening and weekend hours as needed.
Ability to travel locally for client meetings, housing inspections, and community events.
Office environment with potential for remote work as determined by the organization.
