**Job Title: Family Nurse Practitioner**
**Location: iCare Primary Care, Gallatin, TN**
**Reports to: Supervising Physician**
**Job Summary:**
The Family Nurse Practitioner (FNP) at iCare Primary Care will provide comprehensive healthcare services to patients of all ages, ensuring high-quality, patient-centered care in a primary care setting. The FNP will collaborate with physicians, staff, and community resources to deliver preventive, diagnostic, and therapeutic health services.
**Key Responsibilities:**
- **Patient Care:**
- Conduct comprehensive health assessments including patient histories, physical examinations, and diagnostic testing.
- Develop and implement patient care plans, including diagnosis, treatment, and follow-up.
- Prescribe medications and other therapies as needed.
- Provide patient education on health maintenance, disease prevention, and management of chronic conditions.
- **Clinical Duties:**
- Interpret and evaluate patient data to determine management and treatment.
- Collaborate with physicians and other healthcare professionals to ensure continuity of care.
- Monitor and document patient progress and response to treatments.
- **Administrative Responsibilities:**
- Maintain accurate and up-to-date patient records.
- Participate in quality improvement activities and ensure compliance with healthcare regulations.
- Supervise and mentor nursing and support staff as needed.
- Assist in the development and implementation of clinical protocols and procedures.
- **Community Engagement:**
- Engage with community resources to support patient care and health education.
- Participate in community outreach and health promotion activities.
- **Professional Development:**
- Stay current with medical advancements, treatment protocols, and regulatory changes.
- Attend continuing education and professional development opportunities.
**Qualifications:**
- Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
- Current certification as a Family Nurse Practitioner by the ANCC or AANP.
- Licensed as a Nurse Practitioner in the state of Tennessee.
- Minimum of 2 years of experience in a family practice or primary care setting preferred.
- Strong clinical skills in primary care, including preventive and acute care.
- Excellent communication, interpersonal, and organizational skills.
- Bilingual (Spanish) is a plus.
