At Home By Choice Home Care is seeking a dependable, mature, and highly organized Field Manager to help support our caregivers and clients throughout Northwest Indiana.

This is a hands-on field position for someone who leads by example. The Field Manager checks on client care, supports caregivers, helps new employees succeed, responds to urgent situations, and communicates concerns to the office before they become larger problems.

• Visit clients’ homes to complete quality-assurance checks.
• Check in with caregivers and help resolve routine field concerns.
• Mentor and support newly hired caregivers.
• Confirm caregivers are arriving on time and completing assigned duties.
• Report client, caregiver, scheduling, or safety concerns promptly.
• Assist with caregiver introductions and new-client service starts.
• Help cover urgent caregiving shifts when reasonably necessary.
• Maintain accurate notes and communicate regularly with office management.
• Participate in an on-call rotation after completing training.

• At least 21 years old.
• High school diploma required.
• Valid driver’s license and dependable personal transportation.
• Current automobile insurance.
• Must live in Indiana and be able to travel throughout Lake and Porter Counties.
• Caregiving, home care, healthcare, supervisory, or team-lead experience preferred.
• Reliable cellphone with the ability to use scheduling and communication applications.
• Ability to pass required background checks and drug screening.
• Willingness to provide hands-on personal care when assisting with or covering a client’s shift.
• Dependable attendance and punctuality are essential.

• Comfortable visiting clients and caregivers in their homes.
• Capable of handling confidential information professionally.
• Willing to help wherever needed.
• Interested in growing into a meaningful leadership position.

• Schedule: Generally 9a-5p but there may be some after hours and or weekends
• Employment type: Full Time
• Pay: $16-$19 per hour depending on Home Care Experience
• Local travel is required.
• Mileage reimbursement: Reimbursed for mileage during company duties
