Provides Comprehensive case management services for children ages 0 to 18 residing at Hope House, including intake assessment and orientation with parent and child.
Coordinates with adult case management regarding social history of family.
Works with multidisciplinary team for staffing each case.
Maintains necessary documentation, provides role model supervision for children and adults residing at Hope House.
Plans and develops child –centered play therapy activities for children.
Monitors and conducts pre and post assessment of client’s participation.
Maintains ongoing (chronological order) progress notes in each respective case record.
Indentifies client needs and makes appropriate community referrals.
Works with parent-child in identifying case goals and works with them to help facilitate success.
Understands and maintains client confidentiality at all times.
Provides follow up services for clients.
Supervise Child Care Providers and the day to day operation of Hope House child care.
POSITION QUALIFICATIONS:
Must have a bachelor’s degree in human services or behavioral science field.
Should have experience in client casework, preferably in residential setting.
Must possess strong interpersonal skills and communicate well with both children and adults.
Must be available for flexible hours including evenings and weekends when necessary.
Must have concern for the population served and facilitate empowerment based options for clients.
Would prefer someone with working knowledge of services in Central Louisiana area.
Must be able to provide information and confidential referrals to meet client needs.
Maintains client confidentiality.
Have a working knowledge and/or experience in facilitating child-centered play therapy activities.
Must be able to document pertinent information in client case records in timely manner.
Must participate in case staffing with multidisciplinary team.
Must possess a valid Chauffeur’s Drivers License or be willing to obtain one.
RESPONSIBLITIES:
Provide for intake of new child residents (ages 0 to 18) to the program.
Complete admission intake assessment and obtain pertinent information for case record within 48 hours of intake (following Monday on weekends).
Work with parent on family social history.
Provide orientation for parent and child to the children service programming.
Review children center guidelines for participation.
Provide individual case management for each children age 0 to 18
Complete family history questionnaire identifying any history of problems with child.
Work with Hope House Adult Case Management on filling in any gaps in social history.
Update case plans as needed and participate in multidisciplinary case staffing.
Identify age appropriate goal areas with child and mom – set timelines.
Meet with each child and mom a minimum of once per week.
Work with Hope House Case Management on discharge planning for each family.
Act as an advocate on behalf of residents; assist them in accessing local systems for assistance and specific needs – confidential community referrals.
Maintain progress notes in chronological order per case.
Identify any issues or concerns at case staffing meetings. Position is a mandated child abuse reporter. Follow Hope House protocol for reporting.
Facilitate safety planning with each child and parent.
Facilitate Child Centered Play Therapy Activities
Plan and implement age appropriate child centered play therapy activities four times weekly between the hours of 3 p.m. and 5 p.m.
Work with RPSB Teacher in coordinating tutoring and children services activities.
Document progress of child participation in activities in case record.
Seek community partnerships to enhance activities that will expose Hope House children to the arts, music, theatre, etc.
Facilitate age appropriate activities that focus on visual, cognitive, adaptive, motor, social, and communication for children not enrolled in school ages 3 to 5 at least two times weekly from 10 a.m. until 12 noon.
Facilitate pre and post testing or assessments for each child enrolled in children service programming.
Work with approved volunteers generated through Volunteer Coordinator and supervise them while working in children programming.
Maintain grant reporting documentation and statistics required for child centered services.
Supervise Hope House Child Care Provider and Services
Develop curriculum and schedule for child care activities and supervise implementation.
Work with Child Care Providers in coordinating child care activities.
Ensure the safety and well being of all children put in child care.
Coordinate and conduct child care staff meetings and training.
Review and approve Child Care Provider time sheets, leave request.
Review Child Care logs, sign ins, and records to confirm they are accurate and current.
Plan for future growth of program
Establish need based on statistics.
Develop plan of action.
Present ideas to Executive Director.
Assist with public relations and outreach efforts
Make presentations as assigned.
Assist with special events as requested.
Maintain professional credentials
Attend appropriate seminars , conferences, etc. as available and as funding permits.
Maintain any continuing education hours required by are of degree.
Participate in staff development and attend required staff trainings.
Assist with on-call responsibility
Rotate on call responsibility with other designated professional staff.
Maintain appropriate documentation required for on call staff.
Coordinate with Executive Director and Supportive Service Manager
Bring to the attention of the Executive Director and/or Supportive Service Manager any problems with clients, staff or volunteers.
Perform all other duties as assigned by the Executive Director and/or Supportive Service Manager
