We are seeking a Home Health Physical Therapist to join our team! You will plan physical rehabilitative plan of cares to improve patient quality of life and allow for patient's reintegration into the community.

Responsibilities:

• Make home visits to patients
• Develop an appropriate PT plan of care based on assessing patient
• Perform various diagnostic tests such as range of motion, transfer status, ambulation assessment etc...
• Consults with the physician in the development of the therapy plan of care
• Treats patients to relieve pain, develop and restore function, and / or maintain maximum functioning using acceptable standards of practice and evidence based techniques

Pay is $95-$119 PER VISIT

Full Time, Part Time and PRN postions available.
