Support to enhance opportunities for meaningful adult activities and skills acquisition in
community integration and independence.
To include management of financial and personal affairs and awareness of
health and safety precaution;
Support community exploration aimed at discovery of new and emerging
interests and preferences, new relationships;
Support to build community membership;
Implementation of a home therapy program under the direction of a
licensed clinician;
Opportunities for the person to seek employment and vocational supports
to work in the community in a competitive and integrated setting;
Training and support to coordinate or manage tasks outlined in the Health
Care Management Plan, if applicable;
Assistance in performing Personal Care, household, and homemaking
tasks that are specific to the needs of the person, except that this may not
comprise the entirety of the service;
Progress notes, written on a per visit basis.
Progress in meeting the specific goals in the ISP and Plan of Care
addressed on the day of service
Atypical health or behavioral events or change
Evidence of all community integration and inclusion activities and
response to the “what did the person like about the activity?” and what
did the person not like about the activity?”
The start time and end time of services and DSP’s signature;
Matters requiring follow-up on the part of the Waiver Service Provider or
DDS
Coordinating transportation to participate in community events consistent
with this service.

DOCUMENTAION:
Documentation is a key component and requirement as a DSP.
Writing quality notes is very important.
Documentation should be done on the same day that services are provided.
COMMUNICATION:
Ability to effectively communicate orally and in writing with internal and external
business professionals
Communicates with the person, family, support staff and other external bodies
appropriately
