 Support to enhance opportunities for meaningful adult activities and skills acquisition in
community integration and independence.
 To include management of financial and personal affairs and awareness of
health and safety precaution;
 Support community exploration aimed at discovery of new and emerging
interests and preferences, new relationships;
 Support to build community membership;
 Implementation of a home therapy program under the direction of a
licensed clinician;
 Opportunities for the person to seek employment and vocational supports
to work in the community in a competitive and integrated setting;
 Training and support to coordinate or manage tasks outlined in the Health
Care Management Plan, if applicable;
 Assistance in performing Personal Care, household, and homemaking
tasks that are specific to the needs of the person, except that this may not
comprise the entirety of the service;
 Progress notes, written on a per visit basis.
 Progress in meeting the specific goals in the ISP and Plan of Care
addressed on the day of service
 Atypical health or behavioral events or change
 Evidence of all community integration and inclusion activities and
response to the “what did the person like about the activity?” and what
did the person not like about the activity?”
 The start time and end time of services and DSP’s signature;
 Matters requiring follow-up on the part of the Waiver Service Provider or
DDS
 Coordinating transportation to participate in community events consistent
with this service.
 Other duties as may be assigned
DOCUMENTAION:
 Documentation is a key component and requirement as a DSP.
 Writing quality notes is very important.
 Documentation should be done on the same day that services are provided.
COMMUNICATION:
 Ability to effectively communicate orally and in writing with internal and external
business professionals
 Communicates with the person, family, support staff and other external bodies
appropriately
