Position Summary: Deliver, coordinate, and monitor services to assist clients in developing stability to return to the community and reach their full potential.
Skill Sets: Demonstrate good oral and written communication. Maintain a supportive and encouraging working relationship with co-workers and clients. Participate in the initiation, coordination, and implementation of the referral plan. Follow-through, monitor, and evaluates activities related to the recovery of the client. Must have basic computer skills.
Physical and Environmental Demands: Visual acuity and manual dexterity are needed for the required paperwork. Frequent talking and hearing are required when communicating with co-workers, families, and other service providers. Physical demands are light with walking, standing, and occasional stooping. Possible exposure to communicable diseases during contact with individuals released from various locations i.e. jails, hospitals, unsanitary conditions, rehabilitation centers, etc.
Job Responsibilities:
1. Provide quality customer service to personnel, persons served, families, and stakeholders.
2. Help maintain the health, safety, and welfare of personnel, persons served, and stakeholders.
3. Ensure the agency is operating under CARF accreditation standards.
4. Ensure the company adheres to the state and federal licensure and regulations.
5. Conduct client screenings, assessments, intakes, and referral services.
6. Provide individual and group services for clients with substance use disorders and other presenting problems in Residential, Intensive Outpatient, and Outpatient settings.
7. Participate in regular treatment team meetings.
8. Prepare treatment plans, write SMART goals and objectives
9. Work as part of a multi-disciplined team with an emphasis on brief, goal-oriented therapy.
10. Consult and educate in the community and with agencies such as law enforcement, and industry community-based services.
11. Clinicians may be required to work an altered schedule and provide clinical services one to two evenings per week. May be required to work a rotating, weekend, on-call schedule.
12. Client Advocacy
13. In collaboration with other staff and case managers, initiate and maintain complete up-to-date client records including but not limited to client files, incident reports, and discharge plans.
14. Collaborate with other Program Managers and facility staff in program development and client consultation. Provides counseling, feedback, and education to all clients in collaboration with other leadership team members.
